I have already filed an official Identity Theft Report with the Federal Trade Commission through XXXX ( FTC Report Number : XXXX ), confirming that my personal information was used without my authorization. As a result of this identity theft, a XXXX XXXX checking account and related transactions were opened in my name without my knowledge or consent. I did not apply for, authorize, or benefit from this account in any way. The account is fraudulent and the result of criminal misuse of my personal information. I have taken immediate steps to protect my identity and have disputed this account with the relevant institutions and consumer reporting agencies.
