I am a Qualified Medicare Beneficiary ( QMB ) with Medicare and Medicaid. A provider billed me {$770.00} in Medicare Part B coinsurance for an outpatient XXXX infusion claim with date of service XX/XX/year> at XXXX XXXX XXXX XXXX XXXX. Medicare shows the claim was approved but assessed coinsurance totaling {$770.00}. As a QMB, federal law prohibits providers and their agents from billing or collecting Medicare deductibles, coinsurance, or copayments from me.
