I am submitting this complaint regarding a medical collection account that is being reported on my credit report. I dispute the accuracy and validity of this account and request that it be fully investigated and removed from all credit reporting agencies if the company can not provide proper verification. I was not properly notified of this debt before it was placed into collections, and I do not believe the account has been reported fairly or accurately under the Fair Credit Reporting Act ( FCRA ). This negative item has caused damage to my credit profile and ability to obtain fair credit opportunities.

Credit Management Company ( XXXX, PA ) is engaging in illegal debt inflation and fraudulent duplicate reporting on an expired medical debt from XX/XX/XXXX. They contacted me in XX/XX/XXXX claiming a balance of {$2300.00}. They then placed two separate collection lines of {$1100.00} each on my XXXX and XXXX credit reports today, using two different account numbers for the exact same single debt. This deliberate double-billing has tanked my credit score by XXXX points. Furthermore, they reported this to the bureaus within 45 days of initial contact, explicitly violating Nevada law SB 248, which requires a mandatory 60-day notification window before credit reporting. Because the debt is from XX/XX/XXXX, it is past the Nevada statute of limitations and can not be legally enforced.
I am writing to formally dispute the validity of the medical debt mentioned above and to request full validation under the Fair Debt Collection Practices Act ( FDCPA ), 15 U.S.C. 1692g, and the Fair Credit Reporting Act ( FCRA ). Please be advised that I am exercising my right to verify that this alleged debt is accurate and that your company has the legal right to possess my private medical billing information under the Health Insurance Portability and Accountability Act ( HIPAA ).

The account was marked as verified, however, no proper documentation or method of verification was provided as required under FCRA 611 ( a ) ( 7 ). I requested validation and supporting documentation, including billing records and proof of the collectors authority to collect, but none was provided. The account continues to report despite failure to provide adequate verification. I am requesting : Complete method of verification Full documentation supporting the account Immediate deletion if the account can not be fully verified This matter involves potential violations of the Fair Credit Reporting Act and FDCPA.
Collection Management is reporting a collection account for an alleged medical debt with original creditor XXXX XXXX XXXX XXXX XXXX XXXX XXXX in the amount of {$1000.00}. On XX/XX/year> I sent Collection Management a certified written dispute and debt validation request. Delivery was confirmed XXXX XXXX XXXX. The 30-day investigation deadline expired XXXX XXXX XXXX. Collection Management has failed to provide the full name and address of the original medical provider, the date of service, a detailed itemized statement showing how the {$1000.00} amount was calculated, and a copy of any agreement authorizing Collection Management to collect and report this debt. Despite this failure they continue furnishing this account to consumer reporting agencies. Under recent medical debt reporting changes, medical collection accounts under certain thresholds are subject to heightened scrutiny and this account has not been properly validated.
I am not sure what is going on, but I have disputed these accounts multiple times and have never received a response. This has become very frustrating, and I am concerned that my efforts to address these issues are being overlooked. If I made an error in the process, it was certainly not intentional, as I have been doing my best to handle this matter properly and in good faith. Please carefully review the attached letter and conduct a thorough investigation into these disputed accounts. I am requesting that my credit report be made complete, accurate, and fully compliant with all applicable reporting requirements as soon as possible.

In XXXX, I sent written correspondence instructing this company to stop contacting me regarding the alleged debt. Despite that request, the company continued calling throughout XXXX XXXX and has resumed calling again in XXXX. Because of the repeated unwanted calls, I blocked the number on my phone. I initiated a formal dispute under the XXXX Surprises XXXX patient protections process. I am filing a complaint against Credit Management Company regarding an alleged debt that I do not recognize. I have repeatedly requested proper validation of this account, yet the company has failed to provide any documentation to support that this debt is accurate or that they have the legal right to collect it. I am filing a complaint regarding a medical debt in the amount of {$1200.00} that has been placed in collections and is being reported on my credit report. At the time of service, I had both primary and secondary insurance coverage. However, my primary insurance was never billed for this claim. My secondary insurance denied the claim due to the primary insurance not being billed first. As a result, the balance being reported does not appear to be accurate, as the proper billing process through my primary insurance was not completed before the account was sent to collections. I am requesting that this account be fully investigated, properly billed to my primary insurance, and removed from collections and my credit report until the correct balance is determined after all insurance processing is completed.

I am filing a complaint against several debt collection agencies for what appears to be ongoing violations of federal consumer protection laws. After formally disputing the alleged debts and requesting proper validation, the companies did not provide sufficient proof that I am legally responsible for these accounts. They failed to produce key documents such as original agreements, ownership records, account histories, or evidence of their authority to collect. Despite this, collection efforts and credit reporting activity may have continued without proper verification. This raises concerns about potential violations of the Fair Debt Collection Practices Act, including failure to validate debts and use of misleading or unfair practices, as well as the Fair Credit Reporting Act, particularly regarding the accuracy and investigation of reported information.…

I am disputing a medical collection account being reported for {$1200.00} from XXXX XXXXXXXX XXXX. This account has been disputed with the credit bureau ; however, no proper investigation was conducted as required under FCRA 1681i. The account was marked as verified without any documentation being provided. I requested validation of this debt, including an itemized bill, proof of services, and proof of my legal obligation. The company failed to provide any such documentation. The reported balance is inaccurate and unsubstantiated. There is no evidence showing how the amount of {$1200.00} was calculated. Reporting unverified and inaccurate information violates FCRA 1681e ( b ) and 1681i, which require maximum possible accuracy and a reasonable investigation. I am requesting immediate deletion of this account from my credit report, or full validation including itemized billing and proof of liability. I am making this request to ensure the information being reported is accurate and not the result of a mistake, fraud, or confusion. While waiting for proper validation, I ask that you pause all collection activity and reporting related to this account, as allowed by law.

I got a call from ( XXXX ) XXXX on Wednesday XX/XX/XXXX at XXXXXXXX XXXX When I called it back, the person that answers the phone claims to be Credit Management Company. I'll tell them " I got a missed call from this number ''. Based on my caller ID number they ask me to confirm my name, but won't tell me what the call is for. When I ask, they say it's for " a business matter ''. When I ask " what's the business matter? ", they say they can't tell me until they confirm who they're speaking with. Why would I give my personal information if I don't know what it's for? Identity theft is real... I go back and forth with probably 10 of them to try and find out what I'm disclosing personal information for. And nobody will tell me until one of them slipped and said " a collection '' after I had to ask at least 4 times. It's no secret I called many times to try and get answers.…
