They willfully, knowingly, and flagrantly ignored those legal boundaries. 2. The XX/XX/XXXX Barrage and Coercive Calling Tactics On the afternoon of Friday, XX/XX/XXXX, XXXX launched a fast and furious barrage of communication. They bombarded my phone with direct, back-to-back phone calls over and over again from at least three different numbers, using this relentless tactic to trap me into answering. 3. Exploitation of an Account Under Formal Dispute Attached to this update is an official electronic notice from XXXX XXXX XXXX dated XX/XX/XXXX. This letter explicitly proves that Account Number XXXX was forwarded to the Patient Grievance Coordinator for formal review.

I am filing this complaint regarding Credit Systems Intl In for inaccurate credit reporting, unlawful reinsertion of a deleted account, and failure to comply with the Fair Credit Reporting Act ( FCRA ). The account being reported by Credit Systems Intl In DOES NOT belong to me. I have previously disputed this account with the credit reporting agencies, and it was investigated and deleted from my credit file. Despite this deletion, the account has been reinserted onto my credit report without proper notice and without lawful verification. Under the Fair Credit Reporting Act, specifically 15 U.S.C. 1681i ( a ) ( 5 ), a previously deleted account can not be reinserted unless it is properly verified and the consumer is notified in writing within 5 business days of reinsertion. I did not receive any such notification. Additionally, under 15 U.S.C. 1681s-2, furnishers of information have a legal duty to report accurate information.

After cancelling service, I received a final bill containing Balance Due from Billing Plan charges totaling approximately {$540.00}. I never knowingly enrolled in, signed up for, or authorized any billing, budget billing, or average billing plan that would create deferred or retroactive charges upon cancellation. Following multiple known data breaches where my personal information may have been exposed, I reviewed my consumer reports and identified questionable and potentially inaccurate collection reporting associated with this alleged debt. To date, I have not been provided sufficient documentation verifying the alleged account, including a signed agreement, complete billing history, itemized accounting, service records, proof of lawful assignment or collection authority, or consumer-specific documentation establishing that this debt is accurate and belongs to me. Despite the damaging reporting, Credit Systems International continues furnishing information to the consumer reporting agencies without providing competent verification supporting the accuracy and completeness of the alleged debt.

CSI DEBT collectors have continuously called and asked for collections on a medical bill that has not been varified. Even after my wife demanded verification via the Law! This has happened to me several times since I was over 21 years of age, I am XXXX now. No bill has ever been produced or any phone call by original collector or billing department. Then we hear straight from a collection agency and do not know what it's from or that it even existed. This has happened multiple times from covenant hospital in XXXX mi where no bill is made clear to be owed. I am a XXXX veteran and sometimes bills are sent to the wrong place instead of the XXXX XXXX. No notice received. It seems to happen every year, ( not our fault or responsibility ). Billing department employees seem to get lazier every year and do the wrong thing.

Complaint : Claiming i owe over XXXX from Ambulatory ride that I did back in 2024. I had insurance at the time. I don't remember that far off but I would not never call ambulance for myself nor go without insurance. Why would they try to collect now? I never received anything from XXXX XXXX. You see, CREDIT SYSTEMS XXXX IN, has furnished a collection account on my credit report without validating this debt with me beforehand. In fact, the following debt collectors never sent me a dunning letter ( which is mandatory to validate a debt ) within XXXX days of the accounts getting reported ( which is also the initial communication ). Therefore, the accounts listed below must all get deleted from my credit report under XXXX USC XXXX. However, there is XXXX way you can leave the items on my credit report unscathed. And in order to do this, you must produce proof a dunning letter was sent to my address ( within XXXX days of the account getting furnished ).
