I am disputing this debt collection notice because, to my knowledge, I have not received any prior medical bills, statements, or notices regarding this alleged balance. The first notification I received was a debt collection letter stating that I owe {$120.00}. I had active, full XXXX coverage at the time of the alleged service and continue to have full XXXX coverage today. Based on my XXXX eligibility, I should not be personally responsible for this medical bill. If services were provided, the claim should have been properly submitted and processed through XXXX rather than being referred to collections.

This was the debt collectors several attempt to collect the debt from me. For medical debt of XXXX. I requested written documentation of the debt because I had not received any letters or emails, even though the company stated they had already sent them. I did not receive any of those communications. Today, I received very first communication by text message email from the company after contacting them again. During the call, the representative offered to settle the debt for 50 % of the balance, for XXXX and I agreed to the settlement. 30 days from today I have to pay. I asked the representative to email me the settlement agreement before I make any payment. I was told it would be sent, but I have still not received the settlement agreement. I am requesting that the company provide written documentation of the debt and a written settlement agreement before I make any payment via email or letter. I am filing this formal complaint against Revco Solutions , Inc. for a direct and explicit violation of the Fair Debt Collection Practices Act ( FDCPA ), 15 U.S.C. 1692g ( b ). Under federal law, when a consumer submits a written dispute within the statutory XXXX window, the debt collector is legally required to cease all collection activities and communications immediately. They are strictly prohibited from resuming collection efforts until they obtain verification of the debt from the original creditor and mail that validation directly to the consumer. Revco Solutions completely ignored this federal mandate. Timeline of the Violation : XX/XX/year> : I discovered this fraudulent collection file and immediately disputed the validity of the debt. On this same day, I mailed a formal, written dispute and cease-and-desist letter to Revco Solutions via XXXX XXXX Mail.…
REVCOSOLUTIO is reporting a negative collection account on my credit reports with multiple factual inaccuracies and inconsistencies. The Date of Last Activity is different across all three bureaus. The Date Opened differs between XXXX and the other bureaus. The Account Status conflicts because XXXX reports the account as open while XXXX and XXXX report it as closed, but no closed date is listed. The original creditor name is also inconsistent across all three bureaus. XXXX reports XXXX XXXX XXXX XXXX, XXXX reports XXXX XXXX XXXX, and XXXX reports XXXX XXXX XXXX Several key fields are also missing, including last payment, past due amount, account type, payment frequency, and closed date. I am requesting that the furnisher and credit reporting agencies investigate these inaccuracies and delete the account if the reporting can not be fully verified.
I received medical services through a XXXX XXXX XXXX visit on XX/XX/year>. The total charge was {$140.00}. My health insurance paid {$110.00}, and I paid the remaining {$30.00} out of pocket on XX/XX/year>, satisfying the balance in full. Despite the account showing a {$0.00} balance, I later discovered that it had been sent to a collection agency. I was never informed that there was an outstanding balance because there was none. Sending a fully paid account to collections is inaccurate and has caused unnecessary stress and concern about potential damage to my credit. I am requesting that the company investigate why a paid account was referred to collections, immediately remove the account from collections, ensure that no negative information is reported to any credit reporting agencies ( or remove any reporting that has already occurred ), and provide a written explanation of how this error happened and confirmation that the matter has been resolved.
I recently discovered that an alleged medical debt totaling approximately {$570.00} had been placed with a collection agency. According to my medical billing portal, the balances relate to XXXX XXXX visits that occurred in XXXX and were transferred to collections. I did not receive the collection agency 's initial written notice advising me that the debt had been placed for collection, nor did I receive notice of my right to dispute the debt or request validation before collection activity began. Because I never received the required written notification, I was denied the opportunity to timely dispute or verify the alleged debt. I am requesting that the collection agency provide proof that the required validation notice was sent, including the date and address to which it was mailed.…
I am filing a complaint regarding improper billing and collection practices by XXXX XXXX related to a {$15.00} medical charge. I do not dispute the amount owed. My insurer has confirmed the adjudicated patient responsibility. However, I never received a proper written bill for this medical service. I believe it is for laboratory services, not for physician services as indicated in the Revco letter attached. Since I have no bill, I have no way to confirm. I also want to note that I have no direct relationship with either XXXX nor its laboratory. My provider collects the specimen and sends it to the lab ; I do not choose the lab, initiate services with the lab, or have any direct relationship with the lab. Because of this, I rely entirely on proper written billing from the provider or laboratory to understand and pay any patient responsibility. No such proper written bill has been issued. Instead, I received two noncompliant communications : 1.…
I was in the hospital last year for 2-3 hours for chest pain. They ran some tests and said I was fine. I then received a XXXX bill for services. Insurance settled with XXXX XXXX and left a balance of XXXX for me to pay. I would like to see a breakdown of the XXXX bill that I was charged for being in the hospital for 2 hours.
I am a patient of XXXX XXXX and am reaching out regarding an incorrect bill that was issued to me. At the time of my appointment, I was in between jobs but had active XXXX insurance coverage in place. The office was experiencing issues processing my XXXX insurance and subsequently billed me as though I did not have insurance coverage. Since receiving this bill, I have reached out multiple times and provided all information and documentation requested in order for the insurance to be properly processed under my XXXX plan. My most recent correspondence was sent on XX/XX/year>, when I provided the requested documents. Since that date, I have not received any further communication or updates from the office. This balance is incorrect, as the insurance information was provided and should have been processed appropriately. I do not owe this amount, and the billing remains unresolved due to the offices failure to complete the insurance processing despite repeated follow-up on my end.…

Revco XXXX XX/XX/year> {$3300.00} The insurance was active and the services were paid for. They told me that the insurance took the money back. They have to take that up with the insurance company not me. They have been harassing me, which is illegal. About a debt I don't owe. I had insurance that's why I was seen for my appointment, I can't control the insurance company taking their money back for whatever reason. I also have a notice from my former insurance company that they reached an agreement and they will be reimbursing current or former

Between the days of XX/XX/XXXX and XX/XX/XXXX I was contacted over XXXX times via mail about my deceased fathers debt. I have the physical paper proof as well as the scans from my XXXX profile showing the absolutely ridiculous amount of paper mail that is being spam-sent to my mailbox by Revcos medical bill collectors. Ridiculously wasteful, my fathers estate passed outside of probate and after notifying them of this I can not help but feel I am being retributed against for not paying my fathers debt via loss of use of my mailbox because its filled with their junk mail. The dermatology office did not have the correct insurance information on file for me. I explained they had old information and that my coverage changed and was updated. They filed the claim on the old coverage which was not correct. I explained and they supposedly corrected, yet that was not the case. They filed under the old coverage and was denied. It is now too late to appeal with the insurance carrier ( XXXX ). To note, they were paid for the services rendered. It was the non-covered charges, again from the fact they did not file it correctly, that was denied. The credit agency now is trying to collect the debt which I will not pay.
