I am writing to report that my personal information is being used fraudulently and that inaccurate, unauthorized accounts and/or credit activity have appeared on my credit report. Under the Fair Credit Reporting Act ( FCRA ), I have the right to dispute inaccurate information and to have fraudulent items investigated and corrected or removed. I did not open, authorize, or incur the accounts listed, and I believe they are the result of identity theft. I request that the CFPB review this matter and assist in enforcing my rights under the FCRA, as the credit reporting agencies and furnishers have not corrected these errors despite my disputes. Thank you

I am requesting your assistance in resolving a dispute involving XXXX XXXX and a collection agency regarding charges that I believe have never been properly substantiated. I was a XXXX customer for approximately 30 years. In late XX/XX/2025, I transferred my wireless service to XXXX XXXX XXXX after purchasing new devices through XXXX. My new phone and watch were activated on XX/XX/2025, and my XXXX XXXX was transferred. After the transfer, I no longer had access to my XXXX online account, leading me to reasonably believe that my account had been closed. I contacted XXXX and requested copies of my final bills. I was told they would be mailed to me. At no time during that conversation was I informed that any portion of my account remained open or that additional action was required to terminate my service completely. Later, I discovered additional charges. When I contacted XXXX, I was told that although my phone had been disconnected, my account had not been completely closed.…

After its removal, SEQUIUM began reporting what appears to be the same alleged obligation. I dispute the accuracy of this account, the reported balance, and SEQUIUM 's authority to furnish this information. When I terminated my XXXX service, I understood my account to be paid and closed. I never received a final bill showing a remaining balance of {$160.00}. I also never received SEQUIUM 's initial written collection notice informing me of the alleged debt or my right to dispute it. I first became aware of SEQUIUM 's collection activity after reviewing my credit reports.…

We settled for {$230.00}. Each time I call, they confirm, " yes, '' you settled ; the account is closed, but they are refusing to email me documentation. This has been persisting over and over again. Please, any help in enforcing the right to documentation would be greatly appreciated.

I called on XX/XX/year> regarding the remaining line on my account because I switched my other XXXX lines to a new company, they stated it would cost {$30.00} per month. Despite those representations, my concerns have not been adequately resolved. I have consistently maintained that I do not recognize this alleged debt and have requested validation. I do not believe the account belongs to me, and I dispute any assertion that I am legally responsible for the balance.…
