I reported the unauthorized transactions and submitted the required fraud affidavit multiple times, including by XXXX with tracking on XX/XX/year>, to the correct HSA Bank processing address in XXXX XXXX, SD. HSA Bank claims they did not receive the documentation, has repeatedly instructed me to resend the same forms, and has stated there is no supervisor or escalation path and no way to confirm receipt. No provisional credit has been issued, no investigation status has been provided, and no resolution has occurred. This failure to investigate, communicate, or provide provisional credit is a violation of the Electronic Fund Transfer Act and Regulation E. I ate at a diner on XX/XX/XXXX with three of my friends before a hike. We selected the space because of its affordability and service. The bill was a little over XXXX dollars and XXXX cents, and I provided a XXXX tip. I later saw during one of my weekly checks on my account that I was charged XXXX dollars and XXXX cents. On XX/XX/2026 HSA Bank processed a fraudulent claim of {$770.00} from my HSA account. I was not informed of the processing of the claim and it wasnt until I contacted HSA Bank on XX/XX/XXXX that I found out a fraudulent claim had been paid on my card. I was told by HSA bank they would investigate and the balance returned to my account in 7-10 business days. I contacted HSA Bank after 7 business days and they told me it would be another 3-5 business days. I contacted them again after three business days and was told I needed to fill out a claim form.

I opened up a mma account, about five years ago, I don't remember the exact date. Everything was okay until I had a new phone number, I called several times and thought it was taken care of. Was still having trouble, the last time I tried they were saying I tried to sign in too many times. Immediately after the transaction, the bank cancelled my debit card and noticed that it had detected potential fraudulent transactions. However, it has not taken any action to credit the fraudulent debit that took place, despite numerous telephonic conversations and submitting, in a timely manner, the requested written notice via a " Debit Card Transaction Dispute Form '' provided on the bank 's website. While I was traveling outside the United States, the hsabank mailed a replacement debit card to my U.S. address. The card was intercepted before I returned. I strongly disagree with the companys response. This was an unauthorized debit card transaction. I did not make this purchase, I did not authorize it, and I have never had access to the email address used for the transaction ( XXXX ). The bank is relying solely on information provided by the merchant ( XXXX XXXX XXXX ), without verifying that I was the actual cardholder who authorized this transaction. I never received the tickets, never accessed any account, and never benefited from this transaction in any way. Under Regulation E, the burden is on the bank to prove that the transaction was authorized. Simply stating that the merchant provided confirmation is not sufficient proof of authorization. The bank failed to conduct a reasonable investigation and ignored key evidence indicating fraud. I have been very loyal customer of Webster Bank for well over 25 years recently. I have been hit with so many overdraft fee is that I am now in debt and cant even afford to close this bank account.
